Author clarity of purpose certainly helps, although I know I've done my best to be clear with some tricky or touchy matters, and readers/reviewers don't always interpret the way you assume or hope they would. I think there are certain plots and tropes that are so familiar, especially within particular pairings, that readers just expect or read the typical outcomes or dynamics.
